Today, I’d like to introduce a case where we successfully enhanced the nasal line beautifully and naturally—without using implants.

Looking at the pre-surgery photos, the patient originally had a small and slightly low nose, though it was not unattractive. However, a subtly elevated and more defined nose could enhance her features, giving her a more mature and elegant look.

During the initial consultation, we discussed the option of using septal cartilage and a silicone implant. However, as the patient preferred to avoid implants, we proceeded with a technique that achieved a natural yet refined nasal contour without silicone. Many patients hesitate to use implants due to concerns or misconceptions about their safety.

While implant-free rhinoplasty is certainly possible in most cases, a few factors must be considered:

✔ Existing nasal bridge height – A certain level of natural projection is needed to create a well-balanced, seamless transition to the tip.
✔ Skin thickness – If the skin is too thick, it may be more challenging to maintain nasal tip height without an implant.

By carefully assessing these factors, we ensure that each patient receives the best approach tailored to their needs, achieving a naturally enhanced and well-proportioned nose.

Looking at the pre-surgery photos, the nose appears slightly small, while the mouth seems slightly protruded, creating a subtle flatness in the overall facial profile.

Although the patient already had naturally attractive features, the face lacked definition and dimensional balance. To achieve a more sculpted and harmonious look, it was essential to elevate the nasal bridge to create the right amount of facial projection.

In the post-surgery photos, you can see how the mouth appears more balanced, and the side profile, including the E-line ratio, has improved beautifully.

This transformation was achieved by correcting both the nasal structure and the philtrum line during the procedure.

As a result, the lip angle was refined, the mouth appears more proportionate, and the overall profile looks more sophisticated and well-balanced.

In the pre-surgery profile, the glabella (area between the eyebrows) appears sunken, and the nasal tip droops downward, making the nose look longer and less defined.

In contrast, the post-surgery profile shows a smooth, naturally flowing contour from the forehead to the nasal bridge and down to the tip.

The nasal tip is refined and lifted in a way that looks both elegant and naturally proportional, enhancing the overall harmony of the face.

For the nasal tip, we used the patient’s own septal cartilage to provide sufficient height and support.

Standard techniques such as tip suturing or simple tip extension alone would not have provided enough structural stability.

To ensure a strong and lasting result, we reinforced the nasal tip by grafting an additional layer of septal cartilage, creating a stable foundation before proceeding with the rest of the surgery.

At a 45-degree angle, you can clearly see how the face has gained more definition and balance, creating a more refined and beautiful look.

This patient also underwent fat grafting in the following procedure. If there’s an opportunity, we’ll share more details about that in a future post.

Today, I’d like to introduce a case involving a patient with a long, drooping nasal tip.

This patient was particularly concerned about how the nasal tip appeared even longer when smiling, making the nose look more elongated than it actually was.

Many patients with a hooked nose (arrow-shaped tip) or a naturally long nose often share similar concerns.

Let’s go into the details and explain how this can be addressed.

Looking at the pre-surgery photos, you can see that the nasal tip is noticeably long in proportion to the overall facial balance, with a downward angle.

This makes the nose appear even longer than it actually is and gives the midface a lengthened appearance.

In such cases, the key is to lift the nasal tip to create a soft, natural curve. By raising the highest projection point of the tip, we can achieve a semi-upturned nose (straight-to-gently-upturned shape).

This technique helps the nose appear shorter and also creates the illusion of a more balanced and proportionate midface.

From the side profile, the patient’s mouth appears more protruded, and the chin looks larger in proportion.

By elevating the nasal bridge and performing nasolabial angle correction, we were able to improve the angle between the nose and the upper lip (philtrum angle).

As a result, the mouth appears less protruded, and the overall side profile looks more refined and harmonious.

At our clinic, we refer to this as the J-Curve technique, which involves softening the columella and adjusting the philtrum angle to achieve a naturally balanced look.

Simply increasing the height of the nose alone wouldn’t create such a dramatic transformation—it’s the combination of precise adjustments that enhances overall facial harmony.

To better visualize the transformation, we have drawn guiding lines on the image. You can clearly see the improvements in both the nasal line and height.

Now, let’s take a look at the front view.

In the pre-surgery photo, the patient is slightly smiling. When smiling, the depressor septi nasi muscle, which pulls the nasal tip downward, and the levator labii superioris alaeque nasi muscle, which lifts the upper lip sideways, come into action. As a result, the nasal tip appears longer, and the nose seems wider.

For individuals with particularly strong muscle activity, the difference between a resting face and a smiling face can be quite dramatic.

Even after a well-performed rhinoplasty, some patients find that their nasal tip droops again over time or still appears elongated when smiling, causing ongoing aesthetic concerns.

In such cases, nasal tip muscle correction can be performed alongside rhinoplasty to help achieve a more stable and refined result.

We’ve drawn guideline markings to highlight the difference, and you can clearly see how the nose appears significantly shorter after surgery.

At first, smiling may feel slightly unnatural due to the muscle correction, but as time passes, the muscles will adjust and adapt naturally,

Many might think that simply reducing the size of a large nose is a straightforward process. However, the nose is a complex and delicate structure with both aesthetic and functional roles. Since the nose plays a crucial role in breathing, it’s essential to consider both form and function when performing surgery.

In fact, many patients with a humped nose also experience nasal congestion or rhinitis symptoms. If there is an internal structural deviation, it can cause breathing difficulties, which is why a CT scan is necessary to properly assess the condition.

By addressing not only aesthetic concerns but also functional issues, we can provide patients with a comprehensive solution—enhancing both their appearance and breathing comfort.

Rhinoplasty isn't just about reducing nasal height or size—it’s also about ensuring proper function. If there are deviated structures inside the nose, we correct them, and if there is hypertrophic tissue, we carefully remove it. Today, I’d like to introduce a case where we were able to maximize nasal tip extension in patients with a short, bulbous, and upturned nose.

For short noses, especially those with an overly upturned tip, adequate extension of the nasal tip is key to achieving a well-balanced and aesthetically pleasing nasal line.

Proper extension not only enhances the nose’s appearance but also helps create the illusion of a more defined lower face, improving overall facial harmony.

Because of this, the ability to extend the nasal tip sufficiently is a crucial factor in determining the success of the surgery.

As you can see in the photos, the nose is short and upturned, with the nostrils visibly exposed from the front—

commonly referred to as a "piggy nose" or severely upturned nose. Additionally, the overall nose size is quite small.

In cases like this, there is often insufficient septal cartilage available for rhinoplasty.

When this happens, we need to secure additional graft material by using rib cartilage (costal cartilage) or

ear cartilage to ensure adequate structural support and achieve the desired extension and refinement.

Looking at the side profile, you can see that the alar (nostril wings) appear sunken into the cheeks.

With the nasal tip and columella being brought forward, the bulbous tip has been refined, and the previously sunken alar area is now more defined.

This creates a lighter, more open, and balanced appearance, enhancing the overall facial harmony.

Comparing the before and after nasal lines, the previously round and bulbous shape has been refined into a more elegant and feminine contour,

while still maintaining a natural appearance. At the same time, the design was carefully planned to preserve the soft,

youthful charm of the original round shape, ensuring a harmonious and balanced result.

Now, let’s take a look at the transformation from a 45-degree angle.

From the side view, the previously protruding mouth gave a slightly unrefined appearance.

However, with the enhanced nasal volume and structure, the overall look has become much more sophisticated and feminine, creating a more balanced and elegant profile.

These photos were taken in the operating room right before and immediately after surgery for documentation purposes.

Although there is still some swelling right after the procedure, you can already see a significant improvement in the nasal shape.

Today, I’d like to introduce a case involving a humped nose with a bulbous tip.

In most cases, patients with a humped nose tend to have a longer nose with a drooping tip.

However, this particular patient had a unique case of a humped nose combined with a short, rounded, and bulbous nasal tip.

The nasal tip was significantly wide, so I utilized cartilage suturing and added a cartilage graft to support and elevate the tip for improved height.

Additionally, the area between the nostrils (columella) was recessed, disrupting the overall nasal balance.

To correct this, I adjusted it to create a more harmonious and balanced appearance.

The lower part of the columella (the base of the nasal column) was recessed, which made the philtrum appear longer and the mouth look more pronounced.

To address this, I repositioned the columella outward and performed J-curve philtrum line correction.

This significantly improved the appearance of the protruding mouth and achieved a much smoother and more balanced profile, especially when viewed from the side.

Although the patient had a humped nose, it wasn’t very prominent. Additionally, the area between the eyebrows (nasal root) was slightly recessed.

To achieve a smooth and balanced appearance, I used silicone implants to adjust the height and create a more harmonious profile.

As you can see from the side view, the overall impression was quite flat, with the nasal tip appearing slightly bulbous and rounded.

After the surgery, the nose was refined into a more elegant and feminine shape, enhancing the overall profile.

An interesting factor in this case is the strong nasal depressor muscle, which naturally pulls the nasal tip downward, making the nose appear droopy when at rest or especially when smiling.

To effectively correct this, a nasal depressor muscle resection is necessary. This procedure weakens the muscle’s downward pull, allowing the nasal tip to maintain a lifted and balanced position, ultimately improving the overall nasal contour.

By addressing both structural adjustments and muscle function, we can achieve a refined and well-proportioned nasal shape that enhances facial harmony.

This result was achieved solely through tip-plasty, which provides additional support to the nasal tip. By reinforcing the tip structure, the nose naturally appears narrower and more defined from the front view, demonstrating how targeted refinements can create significant improvements without extensive modifications.

The changes in nasal shape are even more noticeable from a lower angle, where the refined structure of the nose becomes more distinct. This view also allows for a closer look at the scar on the columella, which is a natural part of the healing process.

There is no need for concern, as the scar will gradually fade within a few months with proper post-operative care provided by the clinic. With time and the recommended scar management, it will become barely visible, ensuring a smooth and natural-looking result.

When performing alar cartilage binding, as shown in the photo, the columellar strut graft technique can be used to provide additional structural support. This technique involves placing a graft to stand the cartilage upright, effectively elevating and reinforcing the nasal tip.

The patient I would like to introduce today is a male who previously underwent turbinectomy five years ago. Despite the procedure, he continues to suffer from severe rhinitis.

In fact, many patients who visit for a consultation regarding bulbous nose correction tend to believe that their nose will become dramatically smaller and sharper after surgery.

However, it’s important to understand that even with surgery, we are working within the framework of your natural anatomy. That means while improvement is certainly possible, the results may not be as drastically different as those of someone with fundamentally different nasal features.

In particular, patients with bulbous noses often have thicker skin and a greater amount of soft tissue around the nasal tip.

Because of this, it can be challenging to achieve an extremely small and narrow nose shape, even with surgery.

If you look at this patient’s pre-surgery photos, you can see the thickness of the skin and a noticeable amount of soft tissue at the nasal tip, which are common features in bulbous nose cases.

This is the post-surgery view from a 45-degree angle. The bulky appearance of the nasal tip has been reduced, and the tip now looks sharper and more defined.

Since the photo was taken about 2 weeks after surgery, the scar from the alar base reduction is still quite visible. However, this will fade gradually over time with proper healing.

You can clearly see that the nose has become smaller and more refined after surgery.

However, it’s important to note that the result may not be as dramatically slim or sharp as someone who was naturally born with a very small, narrow nose.

In fact, when a nose is altered too drastically, it often ends up looking unnatural or mismatched with the person's facial features.

That’s why it's so important for rhinoplasty to aim for a result that is natural and well-balanced, in harmony with the patient’s unique facial structure and tissue characteristics, rather than trying to force dramatic changes.

Today, I’d like to introduce a case involving a male patient with a bulbous, broad, and low-set nose, particularly noticeable when viewed from the front.

The patient presented with thick skin, wide nostrils, and a low nasal bridge, especially between the eyes, giving the overall nose a flat and heavy appearance.

To improve the nasal height and contour, we used a 4mm silicone implant to elevate the bridge. Additionally, septal cartilage and ear cartilage were used to structure and refine the nasal tip.

To address the drooping of the tip, we performed a depressor septi muscle resection, which helps prevent the tip from pulling downward, particularly when smiling.

There was also a slight dorsal hump on the nasal bridge, which we corrected through a hump removal procedure.

Here’s a basal (bottom-up) view after surgery. The previously wide and spread nasal tip has been reshaped into a more defined and elevated form.

We used a combination of nasal tip cartilage binding and a non-incisional alar base reduction. This method is especially beneficial as it allows for nostril narrowing with minimal visible scarring.

The result is a significantly refined nasal shape, especially when compared to the broad, rounded tip before surgery.
 

In male patients with short, bulbous noses and thick skin, the success of the surgery depends largely on how effectively the nasal tip can be extended and reinforced.

In some cases, if the skin tension is strong or the tissue is too thick, there may be some degree of shortening after healing. When this happens, the nose can usually be re-extended by adding an additional piece of cartilage to restore the desired projection.

Today, I’d like to present before and after photos of a male nose revision surgery.

This patient had previously undergone a procedure using Beko, a type of non-surgical nose enhancement, and semi-permanent fillers.

In the pre-surgery images, the nasal tip appears short and blunt.

Although this patient has high brow bones, the bridge of the nose appears somewhat sunken. For such individuals, raising the nasal bridge can result in a more natural look.

We decided to remove all Beko threads and semi-permanent fillers (although we strive for complete removal, 100% removal may not always be possible).

Below are images of the removed Beko threads. Fortunately, in this case, the tissue adhesion was not severe.

Since the nasal tip was very short, we decided to extend the septum and insert silicone implants.

Here are the post-surgery images. The result is a very natural appearance from the nasal bridge to the tip.

After removing the fillers, there were some irregularities in the nasal bridge, which we smoothed out for a natural look.

We used septal cartilage and ear cartilage to elevate the nasal tip.

We used a 3mm silicone implant.

The wide nasal tip has now been refined into a slim, elegant shape.

The difference is also clear from the front view.

The most significant change is seen from the side view. The nasal tip is now higher and longer, complementing the overall facial appearance.

The patient wanted to make the nose appear slightly shorter because it was a bit long, and had concerns about scars and recovery due to keloid-prone skin.

Since the patient had previously undergone nose filler and Hiko thread procedures, we started by dissolving these materials before proceeding with the surgery.

In the pre-surgery photos, the nasal tip appears droopy.

We performed the removal of the drooping tip muscle to excise the muscle pulling the tip downward.

Since the cartilage at the tip was separated, we also performed cartilage stitching to lift the nasal tip.

Additionally, because the bridge of the nose was slightly lower compared to the brow, we crushed and grafted some cartilage to the brow area.

We also performed surgery to smooth out the hump on the bridge of the nose.

In the post-surgery photos, you can see a significantly higher and more refined appearance.

The nasal tip cartilage stitching has narrowed the nostrils and elevated the tip.

From the front view, the previously wide appearance of the nose has been corrected.

Fortunately, despite concerns about keloid-prone skin, the surgical scars have healed well.

If you want to make the nasal tip appear shorter, raising the angle of the tip can generally make it look shorter and more feminine.

Compared to before the surgery, the nose now appears more feminine while avoiding the appearance of being too long.

For the bridge, we used a 4mm medium-sized silicone implant to achieve a natural look.

We also used ear cartilage in addition to the septal cartilage due to a lack of sufficient septal material.

We performed a slight reduction of a low hump on the bridge.

To address the short nasal tip, we stacked about five layers of cartilage to extend the length as much as possible.

After the surgery, the nasal tip has become longer, significantly changing the overall facial appearance.

The nose now looks more natural, feminine, and refined.

The nasal tip has narrowed and elevated, giving a more defined appearance.

Before the surgery, there was a slight asymmetry in the nostrils due to a deviated septum.

Since perfect correction is challenging, some minor asymmetry may remain.

In the pre-surgery photos, the cartilage at the tip of the nose is spread out, making the nostrils appear flared when viewed from below.

To achieve a more natural look, we performed cartilage stitching to elevate the nasal tip.

From the front view, the low bridge is not very noticeable, but you can see that the septum is slightly deviated.

This is a case of mild septal deviation, which needed to be corrected during the surgery.

Although the deviation is not very noticeable when the nose is low, it becomes apparent as the bridge is elevated.

We used a 4mm silicone implant to avoid excessive height.

Since the septal cartilage was somewhat insufficient due to the low and small nose, we supplemented it with ear cartilage.

​In addition to addressing the spread of the nasal tip cartilage, we also performed non-incision nostril reduction because the nostrils were significantly flared.

After the surgery, the broad nose has been successfully reduced in size.

Although many believe that nostril reduction is essential in broad nose surgeries, it is often possible to improve the appearance with just cartilage stitching.

However, some people are reluctant to undergo nostril reduction due to concerns about unnatural smiles or scarring.

It is worth noting that non-incision nostril reduction techniques are available and can be effective with minimal scarring.

Even if an incision is made, scarring and unnatural results are not inevitable, so there is no need to worry excessively.

In the front view, you can see a significant deviation of the nasal bridge to one side.

Although we strive to correct this as much as possible during the surgery, achieving 100% correction can be difficult, and some deviation may return.

Thus, continuous follow-up is necessary.

From the front view, you can see that the previously curved nasal bridge has been corrected.

In the 45-degree angle view, the central hump has been reduced, resulting in a smoother appearance.

We performed cartilage stitching to narrow the flared nasal tip cartilage. Additionally, we layered septal cartilage to elevate the nasal tip.

In the pre-surgery photos, you can see that the nostrils are visible from the front view and the nostrils are flared.

Additionally, the nasal tip is short, the bridge is low, and there is a bumpy low hump.

Due to a lack of sufficient septal cartilage, we used additional ear cartilage.
 

For a natural-looking bridge, we smoothed out the bumpy hump and inserted a 3mm silicone implant.

To address the wide nostrils, we performed non-incision nostril reduction.

Since this procedure was done internally, there are no visible scars on the outside.

From the front view after the surgery, you can clearly see that the nose has become narrower and the bridge is higher.

From the side view, the previously bumpy appearance has been smoothed out, resulting in a very refined look.
