My Nose Spreads Worse When I Smile — A Finger Check and the Depressor Septi Muscle

2024. 9. 11.

If the tip position differs between resting and smiling, the depressor septi muscle's action is checked. This article organizes the finger-under-nose check and the methods that can be reviewed without raising the bridge.

Key points

  • Comparing photos at rest and while smiling shows the tip lowering slightly toward the lips.
  • A tip that droops or looks wider when smiling can involve the depressor septi muscle among several muscles plus the tip's support structure.
  • Whether the cause is muscular and whether surgical correction fits is judged after directly checking expression change and structure.

"My bridge is low and my tip is blunt, giving an overall unrefined impression. I had no complaints before, but the tip seems to get blunter over time and it stresses me. My face is round so a sharp nose alone wouldn't suit me — I have no plans to raise the bridge. But my already-spread nose gets worse when I smile, so I've come to shy away from laughing freely."

A question that came into KOWON Plastic Surgery. They said it looks worse in photos taken by others from afar than in close-ups, so even group photos became unwelcome. They didn't want a big change from their original face or an obvious surgical look — just to fix the nose that spreads when smiling.

01

What causes a nose to spread when smiling?

First, compare a photo at rest with a photo while smiling. You can notice the tip lowering and dipping slightly toward the lips.

For a clearer check, you can do this.

  • Lightly touch a finger under your nose.
  • Smile in that position.
  • Check whether you feel the tip pushing your finger downward.

This movement can involve facial muscles including the depressor septi muscle under the columella, plus the tip's support structure. Pressing with a finger is only a reference for observing change — not a test that confirms the cause.

02

How is it corrected?

If muscle action is confirmed as the main cause, a method to modulate it can be reviewed. However, treating only the muscle doesn't guarantee the spreading will stop or that no scar will remain — surgical scope must be set including expression function, asymmetry, and recurrence possibility.

What is checked when you want to examine only the tip without raising the bridge is covered in the nasal tip procedure guide.

Summary — look at resting vs. smiling separately

When a spreading nose bothers you, the alae or bridge come to mind first. If it's noticeably worse when smiling than usual, changes in facial muscles and the tip's support structure are checked together. Pressing a finger under the nose while smiling is only a reference for observing movement — not a diagnosis; correction scope is set after direct examination.

Frequently asked questions

Answers to common questions patients ask before a consultation.

Can I check myself whether my nose spreads when smiling?
Touch a finger under your nose and smile — you can feel the tip pushing your finger downward. Comparing photos at rest and while smiling is another way.
Does surgery through the inside of the nose leave a scar?
An approach modulating the depressor septi muscle can be reviewed, but actual incision, scarring, and functional risks differ by surgical method.
Can only the smile spread be corrected without raising the bridge?
If the tip being pulled down when smiling is due to muscle action, it can be reviewed within the scope of addressing that part.

Responses to treatment and recovery progress vary by individual. Outcomes and side effects vary from person to person. Bleeding, infection, swelling and asymmetry may occur. Please consult our medical team before making a decision.
