Choosing a revision rhinoplasty clinic — why contracture happens and checking RIBFIT™ availability
2025. 12. 18.
Why silicone contracture occurs (capsule formation·immune response·infection), why internal structure matters more in revision, autologous rib cartilage harvest methods, and the RIBFIT™ processing process — what to check when choosing a revision clinic.

Key points
- Silicone contracture isn't chance — it's a phenomenon where a capsule formed by foreign-body reaction thickens and contracts, and it arises more easily after surgery that didn't sufficiently account for skin thickness·cartilage state.
- In revision, the internal structure's state matters more than the visible line — what replaces excised or absorbed cartilage must be chosen carefully.
- If septal·ear cartilage was already used, rib cartilage gets considered — and it's worth checking how warping is handled, including whether processing methods like RIBFIT™ are available.
Unlike the excitement of a first surgery, many people visit a revision clinic carrying anxiety.
The nose is a structurally complex organ, so a satisfying line can be hard to meet in a first surgery. A vaguely wished-for shape may not suit the actual features, or inflammatory reactions may appear. Having focused on rhinoplasty for over 23 years, I emphasize to those weighing revision that analyzing the first failure's cause and choosing a suitable material must come first.
This article organizes why contracture occurs, how revision differs from a first surgery, and what to check when choosing a clinic.
01
Same silicone — why did contracture come only to me?
When the capsule formed around silicone thickens and contracts under influences like immune response or infection, contracture can appear. The progression goes like this.
- When an external substance like filler, thread, or silicone enters, the body recognizes it as foreign and creates a protective membrane (capsule) for defense.
- If an excessive immune response occurs in this process, or bacterial infection arises from poor aftercare, the capsule thickens and contracts.
- As a result, the tip skin hardens and lifts upward like an upturned nose — the contracture phenomenon.
It's not simply a matter of chance. It's often the result of surgery pushed ahead without sufficiently considering skin thickness and cartilage state — and the greater the initial tissue damage, the easier inflammation, contracture, tip deformation, or the implant piercing through the skin mucosa becomes. That's why precisely determining bone state and the implant's material·position·size via 3D CT analysis and the like at the planning stage matters.
02
How does revision differ from a first surgery?
What must be weighed more carefully than the visible line's shape in revision is the internal structure's state. This applies especially to those who come for pain or various discomforts rather than disliking the line.
Reopening an already-incised site and rearranging structure demands far more meticulous analysis and focus than a first surgery. It's a demanding task that must also add design harmonizing the features while supplementing complexes.
Especially in second or third surgeries, what replaces cartilage excised or absorbed into imbalance must be discussed carefully. If cartilage has already weakened from inflammation, a material with strong support suitable for the body must be used — otherwise another surgery may follow. Whether a clinic explains such points properly becomes a selection criterion — detailed judgment criteria can be found in the revision rhinoplasty guide.
03
Is autologous rib cartilage harvest painful and hard?
If a problem arose with an artificial implant like silicone, autologous tissue gets considered as an alternative. But if septal or ear cartilage was already fully used in prior surgery, another tissue must be sought.
Among them, rib cartilage is abundant in volume, strong in support, and not easily absorbed — a more-than-sufficient material for rebuilding a collapsed tip. Still, many feel fear hearing that general anesthesia and a chest incision are needed for harvest. At our clinic, it proceeds under sedation anesthesia with only a local-site incision, lowering the pain and scar burden.
04
What is the RIBFIT™ method, and why check it?
Rib cartilage is firm — so if the medical team's skill isn't proficient, a warping phenomenon can appear. That's why a stage of thinning and delicately shaping the cartilage as needed before insertion must precede well. Even so, cases remained where the surface became uneven depending on the patient's skin characteristics or aftercare — leaving something to be desired.
RIBFIT™, developed to overcome that limit, is a method that processes autologous rib cartilage into fine particles, then crafts it into a shape fitted to the individual's nasal structure to stabilize the form. Rather than laying harvested cartilage on as-is, it's newly designed — pursuing a smooth line and form stability together while keeping autologous tissue's advantages. Details on the method can be found in autologous rib cartilage RIBFIT™.
Rather than boasting of such technique, I empathize with each patient's earnestness — so I proceed focusing on just 2 cases a day, not factory-style care.

Summary — check whether the clinic analyzes causes and explains materials
Revision rhinoplasty starts with analyzing why the first surgery failed. Why contracture occurred, what state the internal structure is in, whether remaining cartilage suffices — only after confirming such things are suitable material and method determined. Especially if rib cartilage is needed, please check whether the clinic explains even how warping is handled and whether processing methods like RIBFIT™ are available.
If curious about your nose's revision direction, it can be checked in a 1:1 video consultation.
